Medicare in Mississippi: 2025 Guide

Mississippi has approximately 800,000 Medicare beneficiaries, with Medicare Advantage plans enrolling about 50% of the market. Mississippi has the highest poverty rate in the United States, which creates significant demand for Medicare cost-assistance programs and a high rate of dual-eligible beneficiaries. However, the state has not expanded Medicaid under the ACA, leaving a notable coverage gap for lower-income adults and limiting some forms of cost assistance available in expansion states.

Medicare Advantage Plans in Mississippi

Mississippi's Medicare Advantage market is anchored in its larger urban centers. Jackson, the state capital and largest city, offers the most plan options, with beneficiaries typically able to choose from 20 or more plans. Major carriers active in Mississippi include UnitedHealth, Humana, Magnolia Health (a Centene subsidiary), and BCBS of Mississippi. Gulfport and Biloxi along the Gulf Coast also have moderate plan availability. Some plans in these markets offer $0 premiums with supplemental dental, vision, and hearing benefits — important given Mississippi's high rates of chronic conditions including diabetes, heart disease, and hypertension.

Outside of Jackson and the Gulf Coast, rural Mississippi faces substantial coverage gaps. Much of the Delta region, northeast Mississippi, and other rural areas may have very few Medicare Advantage plans available, and the closure of rural hospitals in recent years has reduced provider network depth even for plans that do serve these counties. Beneficiaries in rural Mississippi should carefully verify that their local providers — including primary care physicians and any specialists — participate in a plan's network before enrolling. In areas with limited plan options, Original Medicare with a Medigap supplement may provide more reliable access to care, including the ability to travel to Jackson, Memphis, or New Orleans for specialty services. MSIS counselors can help assess options for your specific county.

Because Mississippi has not expanded Medicaid, the state's traditional Medicaid program is more restrictive in eligibility than in expansion states. This affects how many Mississippians can qualify for full dual Medicare-Medicaid coverage and D-SNPs, and contributes to the state's high uninsured and underinsured rates among lower-income residents.

Medicare Savings Programs in Mississippi

Mississippi's Medicare Savings Programs are administered through the Mississippi Division of Medicaid (DOM). The QMB, SLMB, QI, and QDWI programs help low-income Medicare beneficiaries pay Part A and/or Part B premiums, deductibles, and cost-sharing. Because Mississippi has not expanded Medicaid, the pool of Mississippians who qualify for full dual Medicare-Medicaid enrollment is more limited than in expansion states — full Medicaid eligibility in Mississippi follows traditional, more restrictive income and asset criteria. However, MSP income limits are set separately by CMS and may be higher than people expect; many Mississippians who believe they won't qualify are surprised to find they are eligible for at least one MSP tier.

Given Mississippi's status as the poorest state in the nation, Medicare Savings Programs and Extra Help are critically important resources for a large share of the state's Medicare population. To apply, contact the Mississippi Division of Medicaid or a local MDHS (Mississippi Department of Human Services) office. MSP enrollment automatically triggers Extra Help eligibility for Part D costs. MSIS counselors can assist with applications at no cost and help identify all programs for which a beneficiary may qualify.

State Pharmaceutical Assistance in Mississippi

Mississippi does not have a State Pharmaceutical Assistance Program (SPAP) to supplement Medicare Part D drug coverage. Mississippians who need help with prescription drug costs should apply for the federal Extra Help (Low Income Subsidy) program through the Social Security Administration. Qualifying for a Medicare Savings Program automatically grants Extra Help status. MSIS counselors can also help identify pharmaceutical manufacturer patient assistance programs for specific medications and assist with Extra Help applications at no charge. Given Mississippi's high chronic disease burden and the resulting medication costs, Extra Help enrollment can represent substantial savings for eligible beneficiaries.
